As a Medicaid Waiver Provider, we provide services on the Community Integration Habilitation Waiver (CIH) and Family Supports Waiver (FSW).

Our approved supports provided under the Community Integration Habilitation Waiver (CIH) waiver are:

  • Residential Habilitation Services (RHS)
  • Day Habilitation Individual (DHI)
  • Wellness
  • Transportation
  • Participant Assistant Care (PAC)

Supports provided under the Family Supports Waiver (FSW) are:

  • Day Habilitation Individual (DHI)
  • Transportation
  • Respite
  • Participant Assistant Care (PAC)

Located in the heart of Cicero, Indiana we have 6 residential locations serving 8 individuals with support within their home.  MMI Residential purchased 3 duplexes to provide affordable housing, community involvement, staffing support as well as creating a family-like atmosphere to individuals with intellectual and developmental disabilities.

MMI Residential also provides support to individuals residing at home with their families. Throughout Hamilton County, we are currently providing this support to 7 individuals. We are approved to provide Medicaid Waiver Support in the following counties:

  • Boone
  • Hamilton
  • Madison
  • Marion  
  • Tipton

MMI’s hardworking and dedicated Direct Support Professionals (DSP’s) provide a variety of support in the home and the community. The support varies for each individual and their specific needs.

Typical support requested and provided are:

  • Assisting with Activities of Daily Living (ADL)
    • Self-care & Hygiene (showers, dental hygiene, clean clothes)
    • Nutrition (meal preparation, safe food handling & storage, grocery shopping)
    • Health Care (medication administration, doctor appointments, therapist appointments)
    • Household Maintenance (cleaning, laundry, household budget and financial education)
    • Adulting (involvement in the community, maintaining employment (if applicable) as well as education on their individual rights and “Abuse, Neglect or Exploitation” education)
